This notice describes how medical information about you may be used and disclosed and how you can get access to this information under the Health Insurance Portability and Accountability Act (HIPAA). Please review it carefully.
Our commitment
Happy Labs and the physicians providing care through the independent clinician network are required by law to maintain the privacy of your protected health information (PHI), give you notice of our legal duties and privacy practices with respect to your PHI, and follow the terms of the notice currently in effect.
How we may use and disclose your PHI
Treatment. To provide, coordinate, or manage your medical care, including sharing information with the physician reviewing your intake, the licensed pharmacy dispensing your prescription, and the CLIA-certified lab processing any at-home test kits.
Payment. To bill and collect payment for services and treatments, including verifying eligibility and coordinating benefits where applicable.
Health care operations. To support the operation of our practice, including quality assessment, clinician credentialing, patient safety activities, and business planning.
Required by law. When we are required to do so by federal, state, or local law, including mandatory reporting of certain conditions to public health authorities.
Uses and disclosures that require your authorization
We will not use or disclose your PHI for marketing, sell your PHI, or share psychotherapy notes without your written authorization. You may revoke a prior authorization in writing at any time, except to the extent we have already relied on it.
Your rights
Access. You have the right to inspect and obtain a copy of your PHI held in our designated record set.
Amendment. You may request an amendment to PHI you believe is inaccurate or incomplete.
Accounting of disclosures. You may request a list of certain disclosures we have made of your PHI.
Restrictions. You may request restrictions on certain uses and disclosures of your PHI. We are not required to agree to every request but will accommodate reasonable requests where possible.
Confidential communications. You may ask that we communicate with you in a specific way (for example, by a particular phone number or email address).
Paper copy. You have the right to receive a paper copy of this notice on request.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us at support@happylabco.com or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
Changes to this notice
We reserve the right to change this notice and to make the revised notice effective for PHI we already have about you as well as any information we receive in the future. The current notice will always be posted on this page with its effective date.
